ABSTRACT: Hepatitis B remains an important global health problem, with more than 600 000 deaths attributable to hepatitis B each year. Liver transplantation remains the only hope for many patients with end-stage liver disease or primary liver cancer due to hepatitis B. This review article details the evolution of prophylaxis regimes to prevent hepatitis B reinfection of grafts, current prophylaxis regimes and the treatment of recurrent hepatitis B in grafts. The issues surrounding transplantation of hepatitis B core antibody positive donor livers are also discussed.
